What Happens When You Have the Injection

The mechanism is simple and worth knowing. When a muscle contracts, it receives a chemical signal called acetylcholine at the neuromuscular junction, the point where nerve meets muscle. Botulinum toxin type A blocks the release of that signal. The muscle cannot receive the instruction to contract. It relaxes. And the skin above it, which was being repeatedly folded by that contraction, stops being folded.

Think of it like paper. Fold it in the same crease a hundred times and the fold becomes permanent. Stop the fold from forming, and the paper stays smooth. A 2023 paper in Research, Society and Development (DOI: 10.33448/rsd-v12i6.42223) describes this precisely: the toxin prevents the fusion of acetylcholine vesicles with the cell membrane, reducing its release. Temporary muscle relaxation follows, lasting until the body regenerates nerve endings, typically within 3 to 6 months.

The Distinction That Changes Everything: Dynamic vs Static Wrinkles

Here is the conversation most patients have not had before they come in, and it is the most important one.

Dynamic wrinkles appear only during expression: the lines between the brows when frowning, crow's feet when squinting, forehead lines when raising the eyebrows. At rest, they disappear. Anti-wrinkle injections are highly effective here because they address the exact cause.

Static wrinkles are visible at rest. They have been formed by years of repeated movement, sun damage, or collagen loss, and the skin now holds the line regardless of expression. Anti-wrinkle injections can soften early static lines and prevent dynamic ones from becoming static. They cannot fill a line that has already been etched deeply into the skin.
